How to get started.

 

Before scheduling an initial intake appointment or after your initial consultation, please reach out to my administrative assistant, Rachel Allen, at gracefulassistant84@gmail.com or call her at (502) 432-9228.

 

She will verify your benefits, and if I am an in-network provider, she will email you a link to get set up in my electronic health record, TherapyNotes.

 

In TherapyNotes, you will complete all of the forms, upload your insurance card, and provide a debit/credit card to keep on file. You can also pay with check or cash.

If you do not want to use your insurance or I am an out-of-network provider, we are happy to provide you with a superbill. 

You may also use an HSA account as a form of payment.
